Here are the forms needed to register your child for NOAH Basketball--we need 1 set of completed forms for each player:- NOAH Athlete Information, Release and Medical Authorization—page 14 of Handbook
- NOAH Parent/Athlete Handbook & Code of Conduct Acknowledgement Form—page 15 of Handbook
- Current Physical—online form is available at http://noahjags.org/NOAH/forms/e-physicalexam2011.pdf. You may take this form to your medical provider or use the form they provide.
- Volunteer Program Form-- Volunteer Program--fill out page 2
- Uniform form—need one for every Elementary & JH player-- Uniform Form
- ECC Liability form—for all 10U & 12U players-- ECC Waiver-for 10U and 12U players
FEE Schedule:
Fees: Below is the fee schedule for the 2013-2014 season. There are no increases from last year. You will be charged the rate for the team your child plays on as their primary team--not according to their age.
